Customers, brokers, and carrier teams may request insurance endorsements with different wording and destinations. A request log keeps those instructions tied to their sources and routes the substantive review to authorized insurance personnel.
Capture the request as supplied
Record requester, carrier entity, policy reference, requested document, requested wording, destination, due date, receipt channel, and source link. Do not paraphrase specialized wording into a field that looks authoritative. Attach the original request.
Validate administrative completeness
Check only the fields defined in the carrier's procedure: named entity, destination, policy reference, contact, and required source documents. Flag missing or conflicting values. Never decide whether coverage exists or whether requested language is legally or commercially acceptable.
Route and acknowledge
Send the packet to the designated insurance owner, broker, or adviser. Log acceptance and any follow-up question. Acknowledging the requester should state that the request was received, not that the endorsement will be issued.
Preserve returned versions
Register each supplied document with receipt time, sender, filename, and version. If a corrected file arrives, retain both. Distribution happens only after the carrier's approved review event.
Review open requests
Group the queue by missing source, external response, owner review, ready to distribute, and closed with delivery evidence. Escalate approaching dates with the request ID and secure links, not sensitive policy details in a public channel.
Working standard
The log shows the original request, completeness checks, authorized review, returned versions, and delivery. It does not interpret coverage or provide insurance or legal advice.
