Carrier Driver Qualification Record Index

A carrier driver qualification record index gives an office a consistent map of files, dates, missing evidence, and review ownership.

Make the index answer one question

The first question should be “which driver file needs attention?” Give every driver a stable row with legal name, internal driver ID, hire date, file location, last review date, and file owner. Do not use a driver's phone number as the identifier. The trucking VA for driver file indexing page covers the administrative setup around this index.

Use one row per evidence item

Record the item name, received date, source, expiration or follow-up date when applicable, reviewer, status, and evidence link. A useful status is received, needs review, waiting on driver, waiting on carrier, or escalated. If a file is missing, say where it was searched and when. That makes a later handoff specific rather than repetitive.

Separate organizing from qualifying

An administrative reviewer can spot an empty field, duplicate scan, unreadable page, or date that needs attention. The qualified carrier owner decides whether the record satisfies the applicable requirement. Check current instructions in the Federal Motor Carrier Safety Administration guidance library and save the source used for the review. Never mark a driver qualified just because every row has a green status.

A practical weekly check

Sort the index by oldest open item, then by the next date. For example, “D-118, annual review item, evidence link opens to a blank scan, compliance owner to confirm replacement” should remain open until someone resolves it. Export the exception list for the owner, record the answer in the row, and keep the original scan. The index is a map, not a substitute for the underlying file.

Handle conflicts before qualification review

When names, dates, or identifiers differ between scans, freeze the row at needs review and list both values. Ask the authorized reviewer which source controls, retain both documents, and record the rationale. The FMCSA driver qualification guidance and 49 CFR Part 391 provide the federal reference points; a complete index is not itself a qualification decision.
